Developed a flexible Python registration form for vaccination/immunization with built-in SMS/email notification capabilities. This program can be customized to suit different applications, including managing customer appointments and sending appointment reminders to customers across various industries. For instance, it can be utilized by hair salons, mechanic shops, or auto detailers to inform customers about their upcoming appointments.

The images above illustrate the customer intake form, which is designed to capture a range of customer inputs. This form can be easily adapted for various applications, including capturing customer preferences or requests. The current form collects basic information, but it can be modified to suit specific needs. This specific example was initially tailored for vaccination purposes, it includes a field for capturing the customer's date of birth (DOB), which is relevant for determining appropriate vaccination protocols based on age.

SMS and Email Notifications 

The program uses pandas data frame and the Twilio API for SMS service. Also uses the Tkinter basic GUI layout. The code is available on my GitHub page.
